[10-21-2025] San Bernardino County, CA – One Killed, Another Injured in Semi-Truck Collision on Westbound I-10 at I-15 Interchange in Ontario

A fatal collision involving two semi-trucks shut down the westbound Interstate 10 at the Interstate 15 interchange in Ontario on Tuesday afternoon, October 21, 2025.

According to the California Highway Patrol, the crash occurred around 1:14 p.m. near the transition ramps.

Multiple vehicles were involved, including two big rigs, one of which caught fire after the impact. Witnesses reported that one of the trucks was braking but failed to stop in time, colliding with another semi. A third vehicle may have been struck between them, though that remains under investigation.

Flames were seen coming from the gas tank of one of the trucks before firefighters quickly extinguished the blaze. Four lanes were initially blocked as emergency crews responded. One person was transported to Pomona Valley Medical Center with major injuries, and at least one fatality was confirmed at the scene.

The cause of the crash remains under investigation, and the identity of the deceased has not been released.

Fatalities increased by 17% compared to 2020. Most of those who died were occupants of other vehicles, accounting for 72% of deaths (4,149 people). Truck occupants made up 17% (1,008 deaths), while 11% (631 deaths) were people outside of vehicles, including pedestrians and bicyclists.

Injuries also rose during this period, increasing 9% from 141,613 in 2020 to 154,993 in 2021. Of those injured, 71% (109,981 people) were passengers in other vehicles, 27% (42,164) were truck drivers or passengers, and 2% (2,848) were nonoccupants such as pedestrians or cyclists.

Among all injury categories, nonoccupant injuries increased the most, up 16%, followed by a 13% rise among passengers in other vehicles and a 1% increase among truck occupants.
