[01-15-2026] San Bernardino County, CA – One Killed, Another Injured in Multi-Vehicle Crash Involving Semi-Truck in Ontario

One person was killed, and another sustained major injuries in a multi-vehicle crash involving a semi-truck on the 10 Freeway in Ontario, Thursday morning, January 15, 2026.

California Highway Patrol says the crash was reported shortly after 3:00 a.m. at the 15 Freeway connector.

A preliminary investigation determined that a Kenworth tractor-trailer, carrying a large excavator, was traveling westbound in the No. 3 lane when it crashed into the 15 Freeway overpass. This caused a metal beam to fall onto a Toyota Camry traveling westbound in the No. 1 lane.

A third vehicle, a Honda Accord, also traveling westbound in the No. 2 lane, subsequently collided with the fallen beam.

Firefighters transported a man and a woman to Arrowhead Regional Medical Center for treatment of major injuries. Despite medical efforts, the woman was pronounced dead at approximately 6:52 a.m.

The driver of the Kenworth, identified as 50-year-old Mr. Liefer of Mira Loma, remained at the scene and cooperated with authorities.

CHP reported that alcohol or drug impairment is not suspected to be a factor. The cause of the crash is under investigation.
